I'm not quite sure how practical this is, but it is without a doubt one of the coolest controller mods that I've ever seen. Ben Heck took a run of the mill wireless 360 controller, fixed it up and created a controller that works using only one hand and your knee.
Frankly, it's really hard to describe...I think that the pictures speak for themselves.
